The In Great Care™ Referral Hub is designed to help case managers, CBCs, behavioral health providers, DV programs, and community agencies connect adults to safe Transitional Housing quickly and responsibly.
We provide a fast, professional referral process, clear communication, and trauma‑informed support for every participant entering our program.

We accept referrals for:

- Individuals exiting foster care systems (EFC, PESS, Aftercare)
- Adults experiencing or at risk of homelessness

Our team reviews the referral and contacts you quickly for follow-up.
Step 3 — Intake Scheduling
We coordinate intake time, transportation, and documentation needs.
Step 4 — Orientation & Move-In
Youth receive a full orientation, safety overview, and program introduction.
Step 5 — Ongoing Communication
We send updates, case notes, and progress reports as required.

How do I submit a youth referral?
Referrals can be submitted directly through our online Referral Hub form. Simply provide the youth’s basic information, required documents, and your contact details. Once submitted, our team reviews the referral promptly and follows up with you for next steps.
What youth populations do you accept?
Adults ages 18–35, including individuals exiting foster care, DV survivors, veterans, individuals experiencing homelessness, and adults needing stabilization support.
How fast is the referral process?
Most referrals are reviewed within 24–48 hours.
What documents are required for placement?
We request a safety plan, EFC verification, basic medical/behavioral information, and any relevant court documents. If some documents are missing, we can still begin the intake conversation and work with you to complete the fil.
Do you accept emergency or same‑day placements?
